This is to certify that the above named student' s work at _____________________________ will be accepted for internship credits pending a successful completion of all of the listed requirements below.     Upon completion of all the internship requirements, a grade of   "Pass"   will be given to the student. The student may receive a grade of   "Fail" if the requirements are not met. The student may initially receive an incomplete grade since the work period may exceed the regular term grading period.

• The student must complete and sign this form.
• The student must provide proof that their work has been granted and approved by the company/ institution.
• The student must register for the internship course (CSC-299 or MAT-495) and is responsible for all financial obligations for this course.
• Period of employment must be at least eight consecutive weeks. The exact number of credits agreed upon will depend on a) the period and total hours of the internships b) the complexity of work.
• The student's work supervisor, upon completion of his/her employment, must submit an evaluation of the work performed by the student during said employment. It is the student's responsibility to ask his/ her supervisor to provide the evaluation. The supervisor may use the evaluation form attached or use their company/ institution evaluation form.
• The student must submit a paper (minimum of five pages, very general, including the overall experience gained) on the work that was performed for the above company.
• A grade of "Pass" will only be given if the student successfully finishes the required work as indicated by a positive supervisor evaluation (form attached) and submits the required paper.
