* The student teacher and an evaluator from Lincoln University must sign this form during each site visitation. One copy is kept in student’s portfolio on site while an evaluator from Lincoln University faculty keeps the other copy for his or her own record. The faculty member has to provide copies of the signed and dated form to claim compensation for travel expenses. The Cooperating Teacher or Mentor must follow the same guideline to qualify for any compensation related to student teaching.
